2010-2011 Club Presidents:

 

Please complete the form below for each member of your club who holds one or more of the following 2010-2011 positions (Rotary position starting July 1, 2010):

2010-2011 Club Administration Committee Chair
2010-2011 Club Public Relations Committee Chair
2010-2011 Membership Committee Chair
2010-2011 Service Projects Committee Chair
2010-2011 The Rotary Foundation Committee Chair

We do not need your information (2010-2011 President) as it is already on file.

We would prefer you input all directory information at one time. If necessary, you can submit what you have now and finish another time (if have not yet filled one of the positions listed above with a club member).  You can come back and submit that particular position.  The deadline is June 1, 2010.


Instructions

Please select Club Name and Position, fill in the form below (using your keyboard and mouse) and then hit the Submit button on the bottom left of this page.  When the "Form Confirmation" page appears, click your mouse on "Return to form".  Repeat this process until you finished submitting names (one name for each 2010-2011 position).  Please also use upper and lower case letters when appropriate (example: John Smith is correct, JOHN SMITH or john smith is incorrect).

 

If you are unable to complete this form and would like me to walk you though filling it out over the phone, please e-mail me at district7500@dcortese.com and I will call you to setup a time.

 

We will e-mail you (2010-2011 President) a complete listing of the member information you sent us to check for accuracy after all the clubs have finished their submissions.
